Income Requirement: Household income must be at least 2.5 times the monthly rent.
Debt-to-Income (DTI) Ratio: Applicant’s DTI must not exceed 30%–40% at the time of application.
Amenities: Optional amenities are billed separately and are not included in rent:
- Laundry Center – $50 per month
- Parking – $100 per month
Water Allowance: Water is included up to the allowance shown below. Usage beyond the allowance will be billed to the Resident.
- 1 Bedroom – $45
- 2 Bedroom – $98
- 3+ Bedroom – $124

NOTICE REGARDING INVESTIGATIVE CONSUMER REPORTS

AMT Management, LLC (“the Company”) may also obtain an “investigative consumer report” and/or a "consumer report" on you from a consumer reporting agency. Such a reports may contain information regarding your criminal history, credit history, tenant history, employment history or other information about you. The information may bear upon your character, general reputation, personal characteristics, and/or mode of living. The consumer reporting agency that may prepare an “investigative consumer report” on you for the Company is American Tenant Screen, Inc., 24 S. The information contained in an “investigative consumer report” may bear upon your character, general reputation, personal characteristics, and/or mode of living. Please be advised that the nature and scope of the most common form of investigative consumer report that may be ordered by the Company is an investigation into your tenant/rental history. During such an investigation, American Tenant Screen, Inc. may ask questions about your tenant/rental history to certain knowledgeable individuals and provide response information to the Company.

No Smoking Policy

POLICY:
It is the policy of AMT Management, LLC and associate companies that there will be no smoking in any interior areas of the property.

SCOPE:
This policy applies to all tenants and visitors.

PROCEDURE:
1. If there is evidence or a report of smoking on the premises, the tenant will be issued notice to cure or quit. Tenants must correct their offending problem or vacate the rental property. If the tenant opts to vacate, they will be fined 3x the current rent as early termination penalty (unless
they are month to month).

2. 2nd notice, the tenant will be fined up to $350.00.

3. 3rd notice, a 90-day lease termination notice will be issued, and an eviction lawsuit will be filed.

ADDITIONAL DISCLOSURES

AUTHORIZATION: I expressly authorize verification of information provided in this application from credit sources, credit bureaus, personal references, current and former landlords and employers. I have the right, under Section 606(b) of the Fair Credit Reporting Act, to make a written request to the credit information source for a complete and accurate disclosure of the nature and scope of any investigation. A credit check may take five business days after receipt of completed application.

APPLICATION FEE: A separate application fee in the amount of $50 (each applicant) accompanies this application. Upon approval and acceptance of this application, the application fee shall be applied to monthly rental; OR upon rejection of this application, the application fee shall be refunded to applicant within ten days of rejection, provided that no false, incomplete or misleading statements are provided in this application.

LEASE & PAYMENT: Upon approval and acceptance of my application, I agree to execute a lease in accordance with the terms of the application and to make payment for the balance of the first month's rent and security deposit in the form of a money order or cashier's check. If I fail to execute a lease and/or make payment, the entire application fee accompanying this application shall be forfeited without recourse, as liquidated damages, and split equally between the Landlord and Listing Broker.

OCCUPANCY: Property is to be used as a single-family residence, subject to all applicable zoning laws and all rules, regulations, by-laws, and covenants of any applicable Condo or Homeowner's Association. Occupancy is contingent upon property being vacated by the present occupant. I understand that the use or sale of illegal drugs on the premises shall be grounds for termination of lease and occupancy, without recourse, and that all advance rental payments and deposits shall be forfeited as liquidated damages in the event of said termination.

CERTIFICATION & REMEDY: I certify that all information provided herein is true and correct and that none of the funds listed are proceeds of illegal activities. I understand that my lease or rental agreement may be terminated and the entire application fee and security deposit shall be forfeited as liquidated damages, without recourse, if I have made any false, incomplete or misleading statement in this application.

Current residence inspection is mandatory
Rhino Security Deposit Insurance is required. Do not purchase until instructed by AMT Management.

Application Fee: The $25 application fee is non-refundable.

Home Visit: As a condition of acceptance, I agree to allow a representative of AMT Management, LLC to conduct a home visit of my current residence within 3 days of provisional acceptance.

Home Visit Addendum

Purpose: This addendum outlines the requirement for a home visit as a term of acceptance for the applicant.

Home Visit Requirement: The applicant agrees to allow a home visit by a designated AMT Management representative as a condition for acceptance. The visit assesses the suitability of the living environment in accordance with AMT Management's standards.
Scheduling: The home visit will be scheduled at a mutually convenient time within 3 days of the applicant's provisional acceptance. The applicant is responsible for coordinating with the representative to ensure timely scheduling.
Compliance: Failure to comply with the home visit requirement may result in revocation of the applicant's acceptance. The applicant must provide access to the home and ensure the visit can be completed without unnecessary delay.
Confidentiality: All information gathered during the home visit will be kept confidential and used solely to determine the applicant's eligibility. No information will be shared with unauthorized parties.
Final Acceptance: Final acceptance is contingent upon successful completion of the home visit. The applicant will be notified of their final status within 3 days following the visit.
